At Mind Oasis Clinic, we value our partnerships with referring GPs and strive to make the referral process as simple as possible. Here’s how you can refer a patient to our clinic:
1. Prepare the Referral Letter
The referral letter should include:
- Patient’s full name and date of birth.
- Reason for referral (e.g., psychiatric, pediatric, or psychological assessment/treatment).
- Relevant medical history and current medications.
- Any other important information related to the patient’s care.
- Indicate if the referral is urgent by clearly stating “URGENT” at the top of the referral letter (for cases that require prioritization).
2. Send the Referral
You can send the referral via:
- Email: Please email the referral letter to referral@mindoasis.com.au.
- Fax: Alternatively, fax the referral to 02 9011 6822.
3. Urgent Referrals
For urgent cases, we reserve a limited number of appointments, typically available within 2 weeks. Please ensure the referral letter clearly states "URGENT" and provide the necessary details to help us assess the urgency.
4. Communication with Our Doctors
If you wish to discuss a shared patient with one of our doctors, please leave a message with:
- The patient’s name and date of birth.
- The matter of concern.
- Confirm you have the patient's consent for this communication.
- Provide your work email address for identity verification and an email for the doctor’s reply.
Our reception team will pass on your message to the doctor. Please allow a few days for a response.
