1. Download and Complete the Medical Record Release Consent Form:

Fill Out the Form: Provide the following:
- Your full name, date of birth, and contact details.
- The healthcare provider’s name and contact details to whom the records will be sent.
- The specific date range or medical records you are requesting.
- Your signature to authorize the release of your records.
- Proof of Identification: Attach a valid proof of identification (Driver’s License or Passport) to the completed form.
2. Submit the Completed Form:
- Email the Form: Email the completed form and your proof of identification to document@mindoasis.com.au.
- Ensure all information is completed clearly and legibly, using block letters if needed, to prevent any delays in processing your request.
3. Pay the Administration Fee:
- Invoice Issuance: Once we receive your completed consent form, our reception team will email an invoice to you.
- Payment Method: You can pay the invoice using a credit card.
- Next Step: After the payment is received, the reception team will proceed with releasing your medical records.
4. Records Sent Directly to Healthcare Provider:
- Delivery Process: The clinic will send your medical records directly to the designated healthcare provider or facility listed on the consent form.
- Secure Transmission: Your records will be securely transmitted to protect your privacy.
5. Processing Time:
- Timeframe: The clinic aims to process medical record requests within 7 days.
- Potential Delays: You will be informed of any delays if they occur due to high volume or complexity.
6. Tracking and Confirmation:
- Notification: You may request confirmation once your medical records have been sent.
